The industry standard is a 200-page PDF, a severity column and a debrief where everyone agrees it was thorough. Six weeks later the same findings are open, because nothing in the document said which three things to do first or who owned them.
Every engagement above produces two outputs from one body of work: one page a executive can forward, and the reproduction detail an operator needs. Same findings, same evidence, never two contradictory documents.
